review results: hub delivers to author, reviewer's lead and author's lead

Maintainer decision 2026-11-02 (chat, seq 37102: "sehr gut! ja"), from the jour fixe proposal r1.

Problem: a reviewer sends its review_result only to the addresses it fills in itself. On the morning of 2026-11-02 at least five results reached only the author and had to be forwarded by hand, so finished PRs sat idle.

Rule: the hub delivers every review_result by itself, without the reviewer adding recipients, to:

1. the author of the PR or the holder of the card;
2. the member the reviewer reports to (its lead);
3. the member the author reports to (its lead).

Everyone gets it once, even if they appear in more than one role. A role with nobody above it is simply dropped. Extra mentions by the reviewer stay possible and are added.

Scope: a contract PR (the review_result audience in the events section, KTW entry with the rejected alternative "reviewer fills in the recipients"), then the hub PR with tests: dedup, no lead, cross-team reviewer, and extra mentions. CLI and skill text only if they tell reviewers whom to address; drop that instruction there.
