web: loading and failure screen — header with logo stays, loader centred

Maintainer feedback 2026-10-27 (phone, #/all): while the app loads, or when it cannot load, the page is a blank strip with "Loading…" top-left. Wanted: (1) the header with the Maity logo renders immediately from web/index.html, before any API call, and stays when loading fails (only the parts that need the session wait); (2) the loader (p.page-loading, the six-dot SVG + text) is centred horizontally and vertically in the remaining viewport, on phone and desktop, light and dark; (3) the same layout for the failure state ("did not answer" etc.), with the retry where it exists. Keep it static HTML/CSS in index.html + the stylesheet so it shows before JS runs. Smoke test for the header-before-load and a narrow-viewport check in web-e2e if the harness allows.
