web: an ended session switches to sign-in instead of polling on with 401s

Found in the #406 first pass (pre-existing, not caused by #406): when the session ends elsewhere (expired, revoked, password changed in another tab), the page stays in its signed-in state; every refresh cycle calls /me and /me/notifications?limit=0 again and gets 401 (6 more within three cycles), the header keeps the old user and the status line says 'Could not refresh: Session ended'.

Fix: one place in the web client treats a 401 on an authenticated call as 'session ended' — stop all background work (notifications stream via M.notify.halt from #406, list refresh timers, counts) and render the sign-in page with a short note; no further calls until signed in again. Smoke: a 401 on /me leads to sign-in and no further requests. After #406 lands. CHANGELOG fragment under Fixed.
