product: coordination improvements from the lead's view (to discuss with the maintainer)

Discussion note, not an order. Nobody starts on this before the talk with the maintainer.

What works: three reviewers from different vendors find real bugs, not style nits. The rule that a false sentence is fixed before the merge, together with Keep the Why, makes the contract and the docs reliable. Inbox, cards and roles carry 25+ parallel agents, and the maintainer only decides what is really his.

Friction, each with a possible direction:

  1. Noise: each review reaches the lead three times (author, review lead, lead). Direction: one review_result with several recipients, or a digest for the lead.
  2. Silent dropouts: an agent that stops listening only shows up when someone reads member_not_listening. Direction: a notice to the lead after N minutes when the agent holds a claim or a review order.
  3. Expensive text-only rounds: one sentence costs a full round with three reviewers and an hour of CI. Direction: a lighter path for text-only deltas (for example CI only for docs, or one reviewer for a one-clause delta, under a rule).
  4. Misaddressed requests: review requests go to the lead instead of the review lead. Direction: the CLI suggests the right recipient from the role.
  5. Machine memory: waits get killed under memory pressure, which can silently cost events. Direction: a resumable wait / catch-up from the last seq.

Lead's view: the bottleneck is no longer coding but coordination; that is where the product should improve next.

Proposals for the talk, checked against docs/api.md on main:

    1. Noise. The hub already delivers every review_result to the author and both leads (§8.4, event table). The triple copies come from reviewers sending three separate messages. Proposal, no code: a rule and a skill line that a review result goes once, to the author. Optionally the CLI refuses a second review_result with the same head and task by the same reviewer within minutes.
    2. Silent dropouts. member_not_listening already wakes the member the agent reports to (§5.3). For reviewers that is the review lead; I see it in my audience. The gap is the reaction, not the signal: only the owner can restart an agent. Proposal: after N minutes, as long as the agent holds a claim or an open review order, the hub files one inbox item to the agent's human ("agent X has not been listening for N min, open work: ..."). Needs a contract change.
    3. Text-only rounds. CI already runs only the jobs for the changed paths (changes job in ci.yml), but a comment in hub/ starts the full hub suite including PostgreSQL. Proposal, a rule for the maintainer to decide: a delta that changes only text (comments, docs, KTW) and has at most a few lines after a 3/3 round gets one reviewer instead of three. CI stays as it is.
    4. Misaddressed requests. Proposal: the skill text and the CLI name the review lead as the recipient of review requests; a maity say to a lead with a review-request wording gets a hint. Small CLI change.
    5. Waits under memory pressure. Cause not reproduced: whether a killed wait loses events or the cursor resumes cleanly still has to be checked before proposing anything. First step: a reproduction (kill a wait while a batch arrives, then compare the cursor with the events).
