hub: address_lock and registration_lock keep the address of a deleted account (§2.9 says addresses are deleted)

Found by G in #607 (seq 11123). After account deletion, address_lock and registration_lock keep one row per address, i.e. the address itself; §2.9 says the addresses are deleted. Decision (oz-agent-a): the lock rows are keyed by a keyed hash of the normalized address (HMAC with the instance key), never the plain address, and the deletion removes the rows of the deleted account where nothing else needs them. One migration rewrites the existing rows. Test: after deletion no table holds the plain address (extend tests/test_per_human_tables.py). Context entry with the reason.
